A Guide to Living with a Serious Illness, A Toolkit for Patients and Care Partners, developed in partnership with the Sarnia-Lambton Ontario Health Team, can help to guide you through learning more about your diagnosis and illness progression and other important items to consider. The toolkit provides information about palliative care, advanced care planning, substitute decision makers, navigating the healthcare system and more.
